Payment for your lessons must be received by the SDSS staff by 7:15
pm on the FRIDAY BEFORE THE FIRST DAY OF CLASS
to complete your registration.
• A LATE FEE of $10 will be added to your registration
if payment is received after 7:15 pm on that Friday,
• For registration(s) with more than one session the total fee for
all sessions will be due within 10 business days for your order
to be complete and confirmed. If the amount received is less than
the total fee for all lessons, only the lessons paid for will be
confirmed. All future lessons will be cancelled so other paying
customers are able to register for those class(es) and time(s).

your minor child/ward for participation in the program(s), you will
be waiving and releasing all claims for injuries you or your child/ward
might sustain arising out of the program(s).

I, as the participant, acknowledge, appreciate, and agree that:
1. The risk of injury from the activities involved in this program
is significant, including the potential for permanent paralysis
and death, and while particular skills, equipment, and personal
discipline may reduce this risk, the risk of serious injury does

and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE
San Diego Swim School,their officers, officials, agents and/or employees,
other participants, sponsoring agencies, sponsors, advertisers,
and, if applicable, owners and lessors of premises used for the
activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY,
DISABILITY, DEATH, or loss or damage to person or property, WHETHER
ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the
fullest extent permitted by law.

recognize and acknowledge that there are certain risks of physical
injury to participants in the program(s) and I agree to assume the
full risk such injuries, damages, or loss regardless of severity
which I or my child/ward may sustain as a result of participating
in any of the program(s). I as parent/guardian with legal responsibility
for this participant, do consent and hereby fully release and discharge
San Diego Swim School and its officers, agents, contractors and
employees from any and all claims resulting from injuries, damages
and losses sustained by me or my child/ward, and arising out, connected
with, or in any way associated with activities of any of the programs,
EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest
extent permitted by law.

agreement and that there are no oral representations, statements
or inducements, apart from the foregoing agreement, which have been
made by the parties, and all parties acknowledge that they have

examine this document and obtain separate counsel for advice regarding
this documents execution, prior to execution of the same and
that the terms of this release are contractual and not mere recital.
